My name is Consuela Chapman and I'm a trained health coach, and licensed clinical behavioral health professional.


I have a simple philosophy when it comes to living your best life.  Life provides us the gift of time.  That time should be enjoyed doing what makes us feel happy and healthy, with who makes us feel loved and appreciated and where we feel safe and secure.

Supportive Therapy

Choices professional counseling services can help you manage areas of concern such as anger, depression, anxiety, stress and addiction.  Choices utilizes a person-centered approach. We tailor our services to meet the needs of the individual.  

Telehealth services are available when appropriate.

Choices provides a supportive environment for those early in recovery; participating in relapse prevention counseling and support is a essential component in maintaining one's sobriety.

Individual and Couples

Choices provides a relaxed atmosphere and an opportunity to share without shame.  There's no time like the present to explore your choices and facilitate the change you desire. 

Group Counseling provides a supportive environment to discuss problems and concerns as you work toward your goals.  Groups are offered throughout the year.  Groups offered include:  Eating and Body Image, Women Wellness,  Writing To Heal , and Mental Maintenance.  They are awesome alone or in conjunction with individual therapy.


Coaching is an option for individuals who desire support in clarifying and achieving personal and professional goals.  

Coaching should be considered if you desire help achieving a better-worklife balance, are in a stuck point in your career, and or wish to improve your health, nutrition, or eating habits.   Unlike therapy, there is no diagnosing and coaching generally is not covered by insurance.   Coaching is available for 1 session or multiple sessions.

EAP Counseling

EAP Counseling is a short-term solution-focused counseling model.  Magellan, OPTUM, CIGNA , ComPsych, WellPoint and Blue Anthem are currently accepted.

Clinical Supervision

Clinical Supervision is offered for small groups and individually.   Individual rates are 60.00 an hour.  Group rates are negotiable.
